Company History

Heritage Healthcare Management, Inc. (HHMI), was formed by David B. Beardsley in 1984, for the purpose of developing and managing retirement, assisted living and nursing facilities. HHMI’s first management contract was with Canon Lodge Care Center in Canon City, Colorado. Canon Lodge is a 60-bed skilled nursing facility with two assisted living homes – Care Age House and The Meadows. In 1986, construction was begun on two 72-bed nursing homes – Heritage Park Care Center was located in Carbondale, Colorado and San Luis Care Center was located in Alamosa, Colorado. David Beardsley was an owner and General Partner of each facility. Upon completion, both facilities were managed by HHMI.

In early 1993, Heritage Park Care Center and San Luis Care Center added 14-unit assisted living facilities to their campuses. These assisted living facilities were managed by HHMI.
In August of 1993, San Luis Valley Medical Investors, Ltd., with Beardsley as the General Partner, purchased the Evergreen Nursing Home in Alamosa, Colorado. This 60 bed skilled nursing facility was managed by HHMI. Upon acquiring the facility, major renovations were initiated. These renovations included a major facelift of the interior and exterior of the building and an addition including a laundry and an inpatient and outpatient physical therapy clinic.

In 1994, a 14-unit addition to the Heritage Park Assisted Living Facility was completed, bringing the total assisted living beds in that facility to 28 beds. Also, a 20-bed Alzheimer’s addition and a 2,500 square foot physical therapy addition were completed in 1994. The four nursing homes and three assisted living facilities were sold on August 1, 1996, along with the management contracts.

After the sale of the above facilities, David Beardsley began the development of five new assisted living facilities in Colorado. As of October, 1997, all five of the facilities were open and under management by HHMI. These facilities are located in Burlington, Lamar, Monte Vista, Pueblo West and Lafayette, Colorado. All of these projects have been designed for eventual expansion of services through already existing assisted living, semi-independent housing, Alzheimer’s or nursing care – as the dynamics of each community warrants.

HHMI began managing The Peaks Care Center in Longmont, Colorado in August of 1997. The Peaks is a 95-bed skilled nursing facility with a 22-bed assisted living facility.

In April, 2000, HHMI completed construction of the first phase of a long-term care campus in Goodland, Kansas. This project currently consists of a 30 bed assisted living facility and 6 semi-independent living duplexes. Here again, the project is designed to be expanded, based upon the needs of the community, into additional assisted living, semi-independent housing, Alzheimer’s or nursing care.

In November, 2000, HHMI closed on the purchase of another facility in La Junta, Colorado and April, 2001, purchased it’s sister facility in Garden City, Kansas. HHMI plans to grow further in the Western States area, through acquisition and development of nursing and assisted living facilities.

In August 2002 HHMI purchased Grace Manor Care Center, a 41 bed, skilled nursing facility located in Burlington, Colorado. This facility has been purchased with the intent of replacing the current physical plant with a new facility as part of a campus located at the site of HHMI’s current assisted living located in the community. Land has since been purchased adjacent to Heritage’s current assisted living facility, on which to build a new nursing home.

2002 was the year to begin expansions of several campuses. The Legacy at Burlington added adjacent duplexes for independent seniors. Services can be purchased monthly, though one meal per day is included in the rent. The Legacy at Monte Vista was the first assisted living center of the company to add 12 apartments along with a beauty shop and common areas. Legacy Commons at Pueblo West followed suit with a similar addition of 12 apartments and the beauty shop and common areas. Then in early 2004 The Legacy at Monte Vista also added duplexes for independent seniors.

January 1, 2004 brought an opportunity to the company to manage a non-profit assisted living owned by a community board in Rocky Ford, called Cottonwood Ridge. It was a perfect fit as it allowed the administrator of Evergreen Gardens in La Junta to oversee both and maintains a common tie in these two nearby communities.

Soon after, another Legacy was opened in Sterling, bringing Heritage Healthcare Management, Inc. into the northeastern corner of Colorado. The Legacy at Sterling is a 27-unit assisted living center that was built by another company in 1999 but closed after two years of operation. The building was renovated cosmetically and re-opened in the summer of 2004 as the premier facility in Sterling.

The year of 2005 was a busy one for David and the company. Twenty apartments with a new dining room and common areas were added to Evergreen Gardens in La Junta, along with six duplexes. Wheat Ridge Acres in Goodland, Kansas also added on – 12 apartments and garages for resident vehicles and extra storage of personal belongings. And two new facilities were added, both small and charming in rural communities. They are called The Aladdin’s and are located in Brush and Keenesburg, with 12 and 16 beds respectively. Also in the summer of 2005, an Alzheimer’s Special Care Unit was made available at Grace Manor Care Center.

As of this writing in 2006, several development projects are on the drawing-board: assisted living centers in Woodland Park, Colorado City, and Trinidad. The company no longer manages The Peaks Care Center and ownership has changed for Evergreen Gardens in Garden City, Kansas, so Heritage Healthcare no longer manages it either.
